The Plan provides benefits for diagnostic testing for infertility the same as other medical expenses. For other infertility treatments, there is a $25,000 lifetime maximum per covered participant.

Other treatments include:

• Ovulation induction.

• Insemination procedures (artificial insemination (AI) and intrauterine insemination (IUI)).

• Assisted Reproductive Technologies (ART), including but not limited to, in vitro fertilization (IVF), gamete intrafallopian transfer (GIFT) and zygote intrafallopian transfer (ZIFT).

•Infertility drugs administered in a doctor's office, hospital, outpatient facility or in a Covered Person's home. Lab tests and professional fees for inpatient and outpatient surgery apply to this maximum; however, office visits do not apply to the maximum.

Parenting and other issues related to children, including:  pregnancy, breastfeeding, child care, adoption, child development, parenting toddlers to teenagers, caring for children with special needs, backup care, summer care, and more. LifeCare’s child care specialists provide personalized guidance and will match you with child care providers and other parenting resources in your area or nationwide. (LifeCare also offers a free prenatal kit with helpful books and supplies for employees who are currently expecting.)
